Are there limits to my access to the Doctor?

You have all the access you need. If you really need to talk to me, tell the staff when you called and they will find me. I generally don't interrupt another patient's visit for a phone call unless your call is urgent, but I return calls throughout the day - you won't wait long for a callback.

Choice Care patients have my cell phone and email and are encouraged to contact me directly by these means, although during office hours it's usually best to call the office number. I encourage you to always leave a message - often I can find the answer before returning your call.

What isn't covered by my Choice Care membership?

There are no additional charges for my services. If we order medications or injections you will pay for the cost of the medications or vaccines at my cost, no mark up. If you need lab work your membership covers the office procedure (e.g. drawing blood), but you are responsible for the lab fees from the third party pathology laboratory.

All other services in my office, via phone, fax or email and services performed by my office staff are included, as are my visits to you in the hospital. Services performed by third parties are not covered by your Choice Care membership. Examples of third party services include:
  • Hospital bills
  • Specialists' bills
  • X-rays, CAT scans, MRIs
  • Physical therapy
Bills for third party services are your responsibility. Your health plan or Medicare/MediCal may cover such expenses, or you may have to pay for these services yourself.

Will Dr. Zwelling call me with my test results?

Whenever possible I will personally call you or email you. Sometimes if I am having a busy day my staff will call you in order to get the news of your results to you more quickly. However, if you want to hear from me and only me just let us know and that is what we will do!

How do I contact the Choice Care staff?

You may contact the staff either directly by e-mail and their addresses are available on the website or you may ask them for their business care when you come into the office.

You may call them directly by calling the office and pushing the number 7 or you may dial their extension directly.

The staff is always available for you. In fact, they really appreciate it when you call them directly with either a specific question or in answer to their query.

What happens if I run of my medication?

Please don’t wait until you run out!

Call us 7-10 days before you run out so we can call your pharmacy for a refill. If you are getting samples, call me before you run out. We get fewer and fewer samples lately and it’s not always possible to keep them in stock.

Don’t stop taking you medications under any circumstances. Contact us and we’ll help you solve the problem. Please don’t substitute a medication without talking to me first. Really bad things can happen if you switch to a replacement, take your friend’s pills or just stop taking your medication. You’ve got my email, you’ve got my cell phone, you can call the staff – so PLEASE, no excuses! Call me about your prescriptions. Don’t take unnecessary chances with your meds.

What if I have a complaint?

Call us or write us with any issue you have concerning the office or our services. Hate getting email? Call us and tell us your contact preferences. Perhaps you’d rather get an email than a call on your cell phone during business hours. Tell me! Would you like a seminar on a specific topic? Don’t like the appointment schedule? Whatever the problem, big, small, or even if it seems silly please let us know and we’ll do our best to fix it.
